- 1). Go to the Institute for Safe Medication Practices (ISMP) website at "https://www.ismp.org/" and click on the "report errors" tab.
- 2). Write an email to the ISMP describing the pharmacy mistake. Include information such as your reaction to the drug, whether or not it was a pharmacy error, the patient outcome, the type of practice the pharmacy is located in, both the generic and brand names of the medicine, the dosage method, the way the error was discovered, and your recommendation for error prevention in the future.
- 3). Upload any images that you might have of the medication, the pharmacy, or your reaction to the drug.
- 4). Include your email address with your submission, in case the ISMP needs to contact you. You have the option to include your name in the appropriate space, but you are not required to.
- 5). Click on the "submit" button to send your pharmacy mistake report to the ISMP.
