- 1). Contact the divorce court in your state of residence to obtain the correct form. Forms differ slightly from state to state, although most states have similar forms.
- 2). Fill out general information such as your name, address and Social Security number.
- 3). Provide as much detail about your income as possible. List all sources of income and approximate amounts per week, month and year. Some states, such as Massachusetts, require you to fill out a separate form if you have self-employment income. Do so if required.
- 4). Provide detail about your monthly expenses, including child support expenses such as food and clothing for your children.
- 5). Sign and date the form. Show it to your attorney. Ask him to check it over for completeness. In some states, he may be required to sign and date the form as well.
- 6). Attach pay stubs and tax returns to the form as proof of income. Submit the form to the court with your other divorce paperwork.
