What Form Do I Use to Apply for a L-2 Visa?

    • Visit the U.S. Department of State, Bureau of Consular Affairs' homepage to download Form DS-156, Nonimmigrant Visa Application. Under U.S. immigration laws, Form DS-156 must be used when applying for all non-immigrant visa benefits, including the L-2 visa. For a link to this page, see the Resources section below.

      Complete Form DS-156 as truthfully and as correctly as possible; any inaccurate information may cause your visa application to be denied. You must answer all questions. If there is a question that does not apply to your specific situation, be sure to enter "N/A" in the question response field. Upon completing the form, you must click on the "Continue" button at the bottom of the page, which will generate a completed Form DS-156, which you can then print out.

    Form DS-157

    • Complete, and print Form DS-157, Supplemental Nonimmigrant Visa Application, from U.S. Department of State, Bureau of Consular Affairs' homepage. To complete Form DS-157, see the Resources section below.

      Form DS-157 is intended to provide immigration officers with additional information about you or your background not disclosed on Form DS-156. Although DS-Form 157 is not the primary application document for a non-immigrant visa, it is nonetheless required of all applicants as the information you disclose will enable immigration officials to judge if you are eligible for an immigration benefit or not.

      Note that since Form DS-157 comes in a PDF format, you can directly complete the form online, then print out the completed form. However, you cannot save any information entered on the form. Thus you must print out the form upon completing it.
