- To apply for Social Security Disability certain criteria must be met as established by the Social Security Administration. Applicants must provide military service discharge papers if applicable, a W-2 form or IRS 1040 and Schedules C and SE if self employed, Social Security Number for a spouse and any minor children, and a checking or savings account and bank routing numbers if payments are made through direct deposit.
- A disability report must accompany all Social Security Disability applications. This must include the name, address and phone number of an individual who can assist with medical conditions and the claimant's application. This may be a family member, a medical case worker or a licensed attorney. Also required are a list of medications currently prescribed and the name of the physician who prescribed each, as well as the name, phone number, patient ID numbers -- typically the applicant's Social Security Number in Michigan -- and treatment dates for all physicians.

Michigan offices are located throughout the state to serve customers and assist in the application process. - Although not required, a licensed attorney in Michigan can assist with the Social Security Disability application. Working with an attorney provides applicants the peace of mind of knowing someone is looking out for their best interest. Also, working with the Social Security Administration can become confusing when trying to check the status of a claim or find answers to questions regarding the application process or necessary documentation. If an initial application is denied, the same attorney can represent the applicant at the appeals hearing. An applicant is never responsible for paying an attorney for assistance; the Social Security Administration compensates the attorney if the initial application is allowed or if the appeal is won.
