- 1). Download an application from the Michigan Department of Human Services website. (See Resource 1.) You can also visit the Department of Human Services office in the county in which you live to obtain an application. (See Resource 2.)
- 2). Complete the application for assistance. If you need help completing the application, ask for help at the Department of Human Services office in the county in which you live. Trained staff will assist you.
- 3). Return the application for assistance to the Department of Human Services office in the county in which you live by dropping it off, faxing it or mailing it.
- 4). Provide proof of income, such as paycheck stubs, child support receipts or a Social Security award letter. Provide other information as requested, such as a birth certificate, Social Security card, proof of pregnancy and bank account statements. If you need help obtaining any of these items, ask for assistance at the Department of Human Services office. Turn in all documents as soon as possible to avoid a delay in the processing of your application.
