How to Submit a Claim to United Healthcare Vision

    • 1). Determine whether your vision care provider is in-network or out-of-network within the UnitedHealthcare vision plan. You can find this information on the UnitedHealthcare Vision website. As noted in the Frequently Asked Questions (FAQs) of the website, no claim submission is required for in-network providers. When you make your initial vision appointment, provide your UnitedHealthcare Vision program identification number and the provider can verify your membership before your appointment.

    • 2). Consult your UnitedHealthcare benefit plan to understand your plan's reimbursement terms for out-of-network providers. Depending on your particular plan, you may or may not be eligible for out-of-network reimbursement. You can find whether you will be reimbursed and for how much by reviewing your benefit brochure, logging on to the UnitedHealthcare Vision website under "My Benefits," or calling your benefit representative.

    • 3). Attend your appointment. Whether your vision care provider is in-network or out-of-network, you are responsible for paying any copays and excluded services. If your provider is in-network, your payment will be processed automatically. For out-of-network providers, you must pay upfront at the time of your appointment. Keep your receipt for this payment.

    • 4). If your plan includes an out-of-network benefit, mail your vision service receipt to the UnitedHealthcare Vision Claims Department. Refer to your benefits manual or the UnitedHealthcare Vision plan website for the correct address. Include your name and date of birth, as well as the subscriber's name if not yourself.
