How to Transfer a CNA License From Missouri to Arkansas

    Applying for a reciprocal certificate

    • 1). Complete an Exemption/Reciprocity Request form to apply for a reciprocal certificate. Contact the Arkansas Department of Human Services (DHS), Office of Long Term Care (OLTC) at (501) 682-1807 to obtain the form.

    • 2). Make a copy of your Missouri nurse aide certificate. If you do not have your certificate, then call the Missouri Department of Health and Senior Services at (573) 751-6400 to request a copy. If you have been certified in states other than Missouri, make copies of those certificates also.

    • 3). Submit the form, along with the copy of your Missouri nurse aide certificate, to the OLTC.

    • 4). Wait for a decision from the OLTC. The office must contact the Missouri Department of Health and Senior Services to verify your certificate, as well as your current status on the Missouri nurse aide registry. If the department is satisfied with your status, then your name will be added to the Arkansas Nursing Assistant Registry. If not, you will receive instructions on how to register for the competency test.
