How to Get a Replacement Social Security Card in Miami, Florida

    • 1). Go to your nearest Social Security office. There is one located at 11401 W. Flagler St. in Miami, and another one at 18475 NW 2nd Ave. If neither of these is convenient for you, you can go to the Social Security Administration website's office locator at https://secure.ssa.gov/apps6z/FOLO/Controller. Type in your zip code to locate the nearest office.

    • 2). Gather the documents you must bring to the Social Security office. You'll need proof of identity and citizenship (at least two documents). You can use your passport or birth certificate to prove citizenship. To prove your identity, bring a current driver's license, non-driver identification card or passport. A U.S. military ID card can also work. You must bring original documents, not photocopies.

    • 3). Fill out a form for application for a Social Security card. You can print this form from the Social Security Administration's website at http://www.socialsecurity.gov/online/ss-5.pdf.

    • 4). Bring the form and all your necessary documents to the Social Security office. Your new Social Security card will be mailed to you if your application and documents are in order.
