- 1). Write, call or visit your local department of social services to apply. You may also be able to apply online through your state's Medicaid website. After initiating your application, you will be asked to provide evidence to determine your eligibility. The income guidelines will vary depending upon your state. You will also need to meet the citizenship requirements.
- 2). Complete the application. Fill out all personal and income information. List all assets and debt. You will need to list bank account numbers and provide account balances. If you own a vehicle, it must be included as an asset. Answer questions about your monthly housing expenses. If you have any other debt, such as credit cards or student loans, include the information in your application. .
- 3). Take a pregnancy test. Most states require you obtain proof of pregnancy from your local health department or hospital. After confirming pregnancy at either place, request a signed letter with your estimated due date.
- 4). Submit all documents to the Medicaid office. The application should state the required documents. You typically will need to supply your driver's license, proof of pregnancy and proof of income. After submitting your documents, a case worker will contact you with any questions or request additional information.
