- 1). Enter your last name and case number in the HUD database search (see "Does HUD Owe You a Refund?" under the References below). If you do not have your FHA case number, enter your last name at the time your FHA mortgage was originated. Click "search" and scroll through the results page until you find your case file. Write down the file information including the full name of all borrowers, and the home address, case number, origination date and refund amount.
- 2). Call 800-697-6967 or e-mail sf_premiums@hud.gov to request a refund application. If requested, provide the information you obtained during your search of the HUD database, and give the operator your current address. HUD will mail you either an Application for Premium Refund or an Application Distributive Share Payment, depending on the type of refund for which you are eligible.
- 3). Complete the application as soon as you receive it. Carefully read the instructions and provide accurate information to speed up the process. If additional information or documents are requested, make photocopies of them and include them with your application. Sign the application and have it notarized. Mail the application to:

Washington, DC 20026-3699 - 4). Contact HUD by phone at 800-697-6967 or by postal mail if you do not receive your refund within 180 days of sending off the application. You can call earlier to check the status of the refund or for options if you believe you are due a refund and your name did not appear in the HUD database.
