How to Answer NBI Fingerprint Cards

    • 1). Fill out the biometric information correctly. The information requested on the card includes date of birth, gender, eye color and hair color. Fill the application out in black ink.

    • 2). Obtain certified, rolled fingerprint impressions on the card. You will need to schedule an appointment with your nearest Philippines consulate general or get the help of your local law enforcement office. Whomever assists you in obtaining your fingerprints needs to sign the card and write down his official designation under his signature.

    • 3). Write your name in the appropriate order. Women who are married must write their maiden surname, husband's surname, first name and then their married surname. All others provide one surname followed by their first name.

    • 4). Get passport-style photos taken that are 2 inches by 2 inches with a white background. Staple the pictures to the card in the appropriate box.

    • 5). Enclose the card in an envelope and mail it to Director, National Bureau of Investigation, Taft Avenue, Ermita 1000 Manila, Philippines. Enclose a money order in the amount of Php 200.00 ($4.60 USD as of May 2011) made payable to the director.
